Costa Nerf Wars – Parental Consent Form

ALL PLAYERS AGED BETWEEN 5-16 MUST HAVE PARENTAL CONSENT BY SIGNING THIS FORM AND ENTERING PLAYING AREA WITHOUT AN ADULT – ALL PLAYERS FOR NERF GUN PARTIES MUST BE 5 YEARS OLD OR ABOVE

 

SAFETY RULES

These guidelines are for the safety of all people using our equipment, and it is the sole responsibility of the hirer to ensure they are adhered to. Costa Nerf Wars cannot accept any responsibility for any injury caused to anyone using this equipment negligently, failing to wear the goggles provided or not following the rules provided by our staff.

• Participants are required to wear the supplied goggles AT ALL TIMES correctly whilst playing NERF Games with Costa Nerf Wars.
• Participants are not allowed to fire their nerf gun deliberately at a person’s head.
• Participants are not permitted to fire at close quarters.
• Our rules of fair play will be explained in the briefing prior to the game starting.
• Children will be warned for repeated unfair play.
• Players may be excluded from the game and asked to leave the venue totally at the organisers discretion if they fail to behave and adhere to the safety rules, no refunds will be offered.
• Anyone caught stealing equipment or stock will be prosecuted according to the law.

For security reasons, the organiser or its agents may exclude participants of the event without giving any reasons if they feel they maybe of detriment of other users.

INDEMNITY TO BE SIGNED BY THE PARENT OR LEGAL GUARDIAN OF THE CHILDREN PARTICIPATING IN COSTA NERF WARS ACTIVITIES.

• I, the undersigned acknowledge that I have read or have been made aware of the safety rules pertaining to the participation by my child in a Cost Nerf Wars Nerf Party, and that I understand these rules and accept the risks which are inherent in the participation in the aforementioned game.
• I am aware and understand that participation in the game involves intensive physical and mental exertion.
• I am aware that participation in this activity involves running which may result in slips, trips or falls.
• I understand that redness and/or bruising can occur as a result of participating in a Costa Nerf Wars Party.
• I warrant as Parent or guardian that my child is free from any medical condition that may endanger his/her life or wellbeing or the life or wellbeing of any other participant.
• By my signature hereto, I voluntarily and knowingly assume the risks at issue inherent to this activity in which my child is participating in.

With my signature I accept this disclaimer.
